Digital Printing:

Copy & Duplicating:

All size copying and duplicating jobs are welcome. Our in-house collating and bindery help speed your orders production time.

Health and Dental Insurance Claim Forms:

Dental forms and UB-04 hospital claim forms are available in laser and continuous formats.

CMC-1500 Medicare Insurance claim forms are available in Laser, continuous and snap-out.

*CMC-1500 and UB-04 forms are printed to Government Printing Office standards in OCR red ink.

Letterhead & Envelopes:

Custom printed letterheads and envelopes are produced with laser safe inks available in flat or raised. Commercial envelopes in different sizes with special adhesives are available direct from the mill. This relationship eliminates the middle vendor and reduces costs.

Labels:

We offer labels appropriate for every application and condition. Stock labels compatible with hospital lab systems like Sunquest and Invision are in inventory. If we do not carry it, we will create it on a variety of substrates with multiple adhesives. Direct thermal and thermal transfer labels with the matching ribbons are in stock.

Medical Imaging Products:

We carry stock x-ray jackets and mailers. Custom jackets are available with minimum quantity of 5000. Special pockets and bar coding can be added to most products.

Paper:

North Shore Forms and Labels carries computer paper, copy paper, color copy paper, and laser cut paper custom punched and always available for immediate delivery.

Plastics:

Magnifiers, business cards, Roll-a-Tab cards, membership cards, insurance cards, credit cards, I.D. tags, door hangers, and hotel keys are all available starting with quantities of 100.
